Adding Additional Organization License Fields

Added 2 new fields to the "applications" and "application-summary" objects:

1. provider-name: is the the organization's legal or AKA name provided in congregate care or child placing agency licensing
2. provider-fein: An organizations fein provided in congregate care or child placeing agency licensing

Added 2 new fields to "licenses" type

1. license-type: An enum to describe the type of the license issued.
2. sub-license-type: An enum listing the sub license type if applicable.
