Added provider external identifier and custom field values to Placement Provider endpoint, and how did you hear about us fields to Applications PUT endpoint

Placement Provider Endpoint

The GET /placement_providers endpoint now includes two new fields:

• provider-external-identifier - The external identifier for the provider profile. Returns null if no profile exists or if the identifier is not set.

• provider-profile-custom-field-values - A hash mapping custom field slugs to their values from the associated provider profile. Returns an empty hash {} if no profile exists or if there are no custom field values.

Applications PUT Endpoint

The PUT /applications/:id endpoint now supports updating the following fields:

• how-did-you-hear-about-us - How the applicant heard about the agency. This field was previously only available on the POST endpoint.

• how-did-you-hear-about-us-details - Additional details for how the applicant heard about the agency. Required for the following enum values: transferred_from_another_agency, other. This field was previously only available on the POST endpoint.
